Need preoperative clearance, fast? We can assess a patient’s overall health status and perform necessary medical evaluations and tests before surgery to ensure the patient is medically fit for the procedure.

We understand that in some circumstances a specialist is needed. Our referral process involves the identification and appropriate referral of patients to specialized healthcare providers or consultants when their condition requires expertise beyond the scope of primary care.

Your mental health matters to us. We address mental health concerns, including the diagnosis, treatment, and management of common mental health conditions like depression, anxiety, and stress through counseling/therapy recommendations and medication management.
Wellness Services
Weight Loss
We offer a safe weight loss aid with Semaglutide injections. Semaglutide promotes weight loss from within, as it helps balance hormones and functions in your body. Botox
We offer cosmetic treatments such as Botox injections for wrinkle reduction and other aesthetic procedures aimed at enhancing physical appearance and self-confidence. Botox is a neurotoxin that temporarily relaxes dynamic wrinkles for a refreshed, healthy appearance. Pricing will be per unit. We offer Allergan Botox and Merz Xeomin.
Hormone Optimization Therapy

Christopher “Kyle” Guillory, FNP-C
Christopher “Kyle” Guillory is a native of Iowa, La, where he currently resides with his wife, Jessica, and their three children. He graduated from McNeese State University with his Bachelor of Science in Nursing in 2007. As a nurse, he worked in area Emergency Departments serving the community that he grew up in. He furthered his education at McNeese State University, graduating with his Masters in Nursing in December of 2019. He began his career as a Nurse Practitioner working in the Urgent Care setting, but for the past 2 years has served SWLA in the Primary Care setting.
Collaborating Physician: Jay Marque, MD
Dr. Jay Marque is the founder and Medical Director of Lake Charles Urgent Care. He received his medical degree from Louisiana State University School of Medicine and has been in practice for more than 20 years. He is residency trained and board certified in Emergency Medicine. Dr. Marque is also a decorated US Air Force veteran. He is now the collaborating physician at Lake Charles Primary Care, and looking forward to working with Kyle Guillory to provide the Lake Charles community with an array of health and wellness services.

What ages are treated at Lake Charles Primary Care?
We treat ages 3 months and up. However, both our weight loss and botox services require that you are 18 years of age and up.

Do I have to make an appointment?
Appointments are encouraged for Lake Charles Primary Care at this time to best serve our patients. This will allow us to streamline the check-in process and minimize your overall wait time. Remember to have your ID and insurance card available upon arrival. We appreciate your patience and trust in our services.

Do you take insurance?
Our primary care services will accept most major insurances except for Medicaid. However, our Wellness services do not accept insurance at this time. Please call our facility for more information on pricing.

What do I need to bring to my first primary care appointment with a new provider?
As you get ready for your first primary care visit, bring any historical medical records, immunization records, photo ID and insurance information.

What do I need to bring to my pre-operative clearance?
Your pre-operative visit at Lake Charles Primary Care will be most efficient if you’re sure to bring any relevant paperwork or forms from your surgeons office and copies of any relevant past medical history records.

Are there any requirements for the weight loss program?
Lake Charles Primary Care requires that you are 18 years or older and have a BMI of 25 or higher to join our Semaglutide Medical Weight Loss Program. You will return weekly on the same day of the week for your semaglutide injections. This allows us to provide wellness checks, open a consistent line of communication with you, and help you stay on track with your weight loss goals.

What should I expect from Lake Charles Primary Care’s Botox services?
Botox injections are usually painless since they only require a small needle. Avoid alcohol, painkillers, and certain medications before your appointment to prevent bruising. As always, inform your doctor of any medications you’re taking. Results may be noticeable in a few days but can take up to two weeks. The effects last about four months, so repeat treatment is recommended.
